The West Jefferson Medical Center Community Health Needs Assessment and Report (CHNA 2013) is intended to provide its users
with a further understanding of the health needs of our community. The CHNA 2013, adopted by the West Jefferson Medical Center
Board of Directors, identifies key health needs in our community and actions to address the findings. The CHNA will serve as
a helpful tool to help guide collaborative planning efforts and provide community information on resources that may provide
programs and services to meet the prioritized needs.
The Medical Center offers its deep appreciation to the many individuals who assisted during the course of preparation of the CHNA
2013 including focus group participants, patients and families, the hospital family and community and business organization leaders
and others who took part in interviews and data collection.
